Part II of the DT-0330 form is project specific. It is to be submitted only by those firms who have been short listed by TDOT. Also, TDOT will request the appropriate firms to submit this form.
ALWAYS mail Part II of the DT-0330 form to the TDOT official from whom you receive a letter inviting you to submit it. If you have questions concerning this form, please contact this same individual.
